Remote Health Monitoring

Remote health monitoring can harness digital technologies to provide wrap-around care to people in their own homes and reduce the need for hospital admission.  For a person with complex health conditions, remote health monitoring can assist in better self-care, self-awareness and confidence to handle common exacerbations at home. 

For health care providers, remote health monitoring solutions can provide a consistent, proactive approach to caring for people with the most complex medical and social needs in the community, rather than a reactive one that could end up in hospital admissions.
