Overview of new Reference sets
National Sepsis Programme Refset Ireland
The National Sepsis Outcome Report emphasizes the significant impact of sepsis on patients and the healthcare system in Ireland. It highlights that 40-60% of sepsis cases are under-documented, necessitating increased vigilance for at-risk individuals. The current dataset's accuracy depends on thorough documentation in medical notes and coding practices.
To improve patient identification, the National Sepsis Team plans to use SNOMED terminology to enhance search criteria related to infection, organ dysfunction, sepsis, and septic shock. This approach aims to increase the identification of affected patients and improve outcomes in sepsis management.
Acute Floor Refset Presenting Complaint Ireland
The Acute Floor Information System (AFIS) project datasets will be launched in several HSE hospitals, tracking patients from attendance to discharge. It will initially use SNOMED datasets for diagnosis, past medical history, and allergies within the acute floor setting. The AFIS team is working with the SNOMED release team to publish Irish datasets for the system.
Cork Kerry Community Healthcare Speech and Language Therapy Ireland
Speech and Language Therapy in Cork Kerry is working to standardize practices across all teams and has identified the need to standardize data flows. It has been agreed that SNOMED CT will support this effort. The primary goal is to enhance the quality of KPI data by ensuring reliable and standardized information about SLT services in the region.
EuroHeart Data standards For Acute Coronary Syndrome (ACS) And Percutaneous Coronary Intervention (PCI) Reference Set Ireland
The EuroHeart ACS/PCI data standards, created by the European Society of Cardiology, are designed for quality registries and research. After a successful pilot implementation in HSE Southwest, there are plans to roll out these standards and the associated platform across the country.
